Offer Nationwide Coverage

Haworth’s healthcare provider Priority Health partners with Cigna to offer nationwide coverage for our members. Members receiving care in Michigan will use the Priority network, while members who travel or live outside Michigan can access in-network benefits—including co-pays and deductibles—through Cigna. Our single-provider relationship with Priority Health enables us to pass along healthcare and prescription cost savings and provide our members with access to a vast network of healthcare providers. These benefits help you and your family stay healthier, receive better care, and get more value from your plan.

Tobacco Surcharge

You will be subject to a tobacco surcharge if you or your spouse/domestic partner regularly used tobacco within the past six months. Regular use includes any use of cigarettes, e-cigarettes, cigars, pipes, or chewing tobacco, whether daily, weekly, or monthly. Your status as a tobacco user will only be used to determine your medical plan premiums. You must declare and certify your status and that of your spouse/domestic partner as tobacco users or non-tobacco users. It is your responsibility to respond truthfully.

Which Plan is Right for Me?

CDHP/HSA: The CDHP/HSA (consumer-driven health plan with a health savings account) offers lower bi-weekly premiums and access to an HSA—a triple tax-free account. Money in your HSA is tax-free when deposited, when it grows, and when withdrawn, as long as it is used to cover qualified medical expenses. Haworth will also contribute money to your HSA. 


PPO: The PPO (preferred provider organization) plan makes it easier to estimate your medical costs. For example, there are defined copays for visits to your primary doctor or a specialist. The downside is that your bi-weekly premium is higher than with the CDHP/HSA. Members who elect the PPO plan can also enroll in the Health FSA to set aside money on a pre-tax basis for healthcare costs. 

Spousal/Domestic Partner Surcharge & Domestic Partner Benefits

You will be subject to a $50 bi-weekly surcharge if your spouse or domestic partner is enrolled in Haworth’s medical plan and also has qualifying medical coverage through their employer. You will be asked to certify the current status of your spouse or domestic partner during the Open Enrollment process. Please read the certification language carefully.

Domestic Partner Affidavit and Important Tax Information

If you are covering your domestic partner under Haworth’s plans, you must complete the domestic partner affidavit, which can be found in ADP. The affidavit, along with pictures of documents proving you and your domestic partner share the same address, must be uploaded into ADP within the required time limit. In addition, please note that the IRS requires employers to tax employees on the cost of adding a domestic partner to insurance coverage. This additional tax may add up to a considerable extra cost to you.
